Who is eligible
An organization that transports blind or disabled persons can request a disability placard online. A qualifying organization must use a vehicle at least 50% for daily transport of qualifying disabled persons. You can use this service to apply for a disability placard, or renew / replace your placard. You can request a renewal of a renewable disability placard within 6 months of its expiration. Temporary placards cannot be renewed.

How to
You will need:
- Organization name
- Business number issued by Connecticut Secretary of State
- Vehicle plate number (must have active Connecticut registration status)
- A valid email address or mobile phone number
- Documentation showing your relationship to the organization (for example, letterhead or business card)
- A credit card (for temporary placard transactions only)
- Completed and signed placard application (Permanent Placard Application Form or Temporary Placard Application Form)
- Your Social Security number (optional)
Fees
- Fee to request a temporary placard is $5
- Permanent placards have no fees
- Refunds will not be given unless permitted under law
