Who’s eligible
If your driver’s license is lost, stolen, or damaged/destroyed, you can get a replacement license, known as a duplicate.
If your name or address has changed since you got your last license, there are additional steps to update your address on your new license.
Follow these steps to get a replacement CDL
Step 1: Print and complete the required form
- Complete an application for duplicate of current driver's license or permit (Form 1-B).
Step 2: Make an appointment
- When you replace a driver’s license, please make an appointment at a DMV office.
Step 3: Get your replacement CDL in the mail
- You’ll get a temporary license at your appointment, and your permanent driver’s license replacement arrives through the mail.
- Your new license is only mailed to the mailing address on file with the DMV. If you have mail forwarding, you will NOT receive your new driver’s license.

Note: You can’t request a duplicate CDL by mail or email if your name has changed since you got your last license.
If you cannot request a duplicate license by mail or email. Please replace it in-person at a DMV office, instead.
Request a license by mail or email
To request a duplicate driver’s license by mail:
- Complete a request for a Connecticut driver’s license/identification card by mail (Form B-350)
- Provide a check or money order for $30 made payable to “DMV,” or you can pay the fee by credit card over the phone (please see instructions below). All checks must be drawn on a United States Bank and made payable in United States funds.

Your duplicate credential is mailed directly to the address on file. You receive it within 20 days.
To request a duplicate driver’s license by email:
- Complete a request for a Connecticut driver’s license/identification card by mail (Form B-350)
- E-mail completed form(s) to DMV.CIU@ct.gov
- To pay for your request, a link will be emailed to you within two business days
If there are any issues with your request, we notify you via email. Your duplicate credential is mailed directly to the address on file, and you’ll receive it within 20 days.
