How to
You will need:
- Your first and last name
- Your date of birth
- Your address
- Your Connecticut driver’s license or non-driver ID number, unless you qualify for an exception:
- Certified blind (requires certification of legal blindness)
- Age 80 or older
- Severely disabled (for example, wheelchair bound)
- Unable to get documents required to obtain a driver license or non-driver ID
- Out-of-state resident injured while in Connecticut
- Your Social Security number (optional)
- A credit or debit card (for temporary placard transactions only)
- A valid email address or mobile phone number
- Completed and signed placard application (Permanent Placard Application Form or Temporary Placard Application Form)
- Certification from a medical professional or qualifying veteran exemption documentation
- Power of attorney
- Proof of date of birth of disabled person
Fees
- Fee to request a temporary placard is $5
- Permanent placards have no fees
- Refunds will not be given unless permitted under law

Who is eligible
You can use this service to apply for a disability placard, or renew / replace your placard. Connecticut residents who are blind, disabled, qualifying veterans, or guardian to a disabled minor under age 18, and non-residents under qualifying conditions, can request a disability placard online. You can request a renewal of a renewable disability placard within 6 months of its expiration. Temporary placards cannot be renewed.
