Required documents
To get additional plates, please provide the following:
- Completed request for marker plates (Form K-6).
- Connecticut special financial responsibility insurance certificate (Form R1325e, provided by your insurance company) that lists coverage for the total number of plates that the licensee will have assigned to them.
- Provide a written statement justifying the need for additional plates. Include the number of employees and your average number of service customers in your letter.
