Remove a member
To remove a member from an existing LLC, please provide the following items:
- Sections 1 and 2 only of application for business license (Form K-7)
- Secretary of State document showing the ownership reflected on the above business license application
- Amended operating agreement
- Payment for fee(s): $20 for each member being removed from the license, payable by check or money order made payable to “DMV”
- K-198 Certificate of Business Status and Agent for Service form
Add a member
To add a member to an existing LLC, please provide the following items:
- Sections 1 and notarization only of application for business license (Form K-7)
- Secretary of State document showing the ownership reflected on the above business license application
- License inspection application (Form K-8), new member please complete Section 1, 2, and 5 only
- Amended operating agreement
- Driver’s license of member(s) being added
- Completed privacy rights form (PDF)
- Payment for fee(s): $20 for each member being added from the license, payable by check or money order made payable to “DMV”
- K-198 Certificate of Business Status and Agent for Service form
