Connecticut’s Office of Health Strategy, in partnership with UCONN and the Yale School of Public Health, looks to improve primary care and achieve better care, smarter spending, and healthier people and communities. To set this idea in motion, the state is conducting a survey to assess consumer experiences with physicians from Advanced Networks (ANs) and Federally Qualified Health Centers (FQHCs), which will help CT evaluate the impact of the latest State Innovation Model (SIM) reforms on the primary care experience, assist health plans in rewarding performance under shared savings contracts, and provide data for a public scorecard.
