FOI and Data Requests

Pursuant to Section 1-200 of the Connecticut General Statutes, anyone seeking public information from OHS can request it through the Freedom of Information Portal:

If the request is related to data OHS collects regarding Hospital Inpatient Discharge data, Emergency Department data, Ambulatory Surgery data or All-Payer Claims Database (APCD) data, please complete the appropriate request form. You will be asked to attach the completed request form and submit it by email after filling out your basic contact information in the request portal.
