The following form has been developed to enable state COVID-19 test strategy partners to request Respiratory Collection supplies. CT DPH currently has a limited stockpile available of Respiratory Testing Supplies. We are happy to accommodate your needs within the confines of quantity and expiration. State supplies of Abbott BinaxNOW™ have all been allocated, and the State of Connecticut will no longer supply antigen testing kits. This request form is for respiratory specimen collection materials (no laboratory testing included).
